Needs Assessment for IRIS Units' Web Sites

Thank you for taking the time to complete the following survey. Ideally, we would like you to fill this out with the Webmaster, or with the person who will have primary responsibility for establishing and maintaining the unit’s Web site(s). If there is a contact person for your Web site who is not the unit supervisor or the Webmaster, please provide that name as well.

We are distributing the forms electronically with the hope that you will provide your answers electronically. You will have as much space as you need for each question. Please use a separate form for each Web site that your department maintains. A link will be provided after you submit this form that will bring up another form. Your responses are due by Friday, December 13.
